Who is a Gynaecologic Oncologist?

A gynaecologic oncologist is a specialist in gynecology trained to manage cancers of the reproductive system. Their role is not just limited to performing surgeries but also involves comprehensive consultation, treatment planning, chemotherapy, radiation, targeted therapies, and follow-up care. When to Visit a Gynaecologic Oncologist

Early detection is the key to saving lives. Screening and preventive services include:

  • Pap smear, liquid-based cytology, and HPV DNA testing/genotyping
  • Colposcopy with guided biopsies, LEEP, and conization
  • Thermal ablation and cryotherapy for cervical pre-cancers
  • HPV vaccination for prevention
  • Genetic screening for hereditary cancer syndromes such as BRCA mutations
  • Risk-reducing surgeries for high-risk women with genetic predisposition

Treatment Options

Minimally Invasive and Advanced Surgeries

  • Robotic and laparoscopic hysterectomies for benign and early uterine cancers
  • Sentinel lymph node biopsy for early-stage uterine cancers
  • Retroperitoneal lymphadenectomy for pelvic and para-aortic nodes
  • Hysteroscopy for diagnosis and management of uterine pathologies
  • Complex laparoscopic and robotic surgeries for recurrent disease

Major Open Surgeries

  • Radical hysterectomy with pelvic lymph node dissection, usually done for early stage cervical cancer, which is a complex surgery involving removal of uterus with cervix, parametria and pelvic lymph nodes. Ovarian preservation by transposition is offered in young women.
  • Radical trachelectomy (fertility-preserving), involves removal of diseased lower part of cervix, parametria and keeps uterus intact for child bearing (in select cases only)
  • Cytoreductive & interval debulking surgeries for ovarian cancer, complete en bloc removal of uterus, cervix with both tubes and ovaries, pelvic+ paraaortic lymphadenectomy, peritoneal disease removal and omentectomy
  • Fertility-preserving ovarian tumour surgeries, removal of affected ovary, and uni or bilateral lymphadenectomy with omentum removal (in select cases only)
  • HIPEC (Heated Intraperitoneal Chemotherapy), heated chemotherapy circulation inside abdomen during surgery to treat peritoneal disease
  • NIPEC/EPIC (Normothermic/ Early Postoperative Intraperitoneal Chemotherapy)
  • Radical vulvectomy & vaginectomy, complex surgery for vulvar and vaginal cancers respectively
  • Inguinofemoral lymph node dissection, involves removal of lymph node groups in groin, usually a part of surgery for vulvar/ lower vaginal cancers

If you experience persistent swelling, abnormal urine reports, uncontrolled hypertension, or elevated creatinine levels, it is advisable to consult a nephrologist.

Treatment includes medicines, lifestyle changes, dialysis (hemodialysis or CAPD), and renal transplant in advanced cases.

Dialysis helps remove waste products and excess fluid when the kidneys can no longer do so. It may be hospital-based hemodialysis or home-based peritoneal dialysis.

Low-salt, low-protein diets, regular follow-ups, strict sugar and blood pressure control, and timely vaccinations are essential.
